JAMA is one of the world’s leading medical journals, publishing research that shapes the scientific agenda and public policy around the globe.
Therefore, nobody associated with scientific research can be allowed to have doubts about our culture’s most sacred dogmas, such as systemic racism.
The controversy began when Dr. Ed Livingston, a deputy editor, said on a Feb. 24 podcast that structural racism no longer existed in the United States.
“Structural racism is an unfortunate term,” said Dr. Livingston, who is white. “Personally, I think taking racism out of the conversation will help. Many people like myself are offended by the implication that we are somehow racist.”
The podcast was promoted with a tweet from the journal that said, “No physician is racist, so how can there be structural racism in health care?” The response to both was swift and angry, prompting the journal to take down the podcast and delete the tweet.
Soon, Livingston was out the door.
A week later, Dr. Bauchner addressed the controversy. “Comments made in the podcast were inaccurate, offensive, hurtful, and inconsistent with the standards of JAMA,” Dr. Bauchner said in a statement. “We are instituting changes that will address and prevent such failures from happening again.”

I want to understand how the podcast can cause trauma and pain to Black physicians and patients. Are they as fragile as 3-year-old children?
If they genuinely believe that there is systemic racism, then the podcast would just be an example of it to confirm their beliefs. So I am guessing that they do not believe in systemic racism, but they are emotionally invested in it anyway.
There are at least 3 deaths among my relatives due to medical negligence. My grandfather died of sepsis after his hip replacement parts were apparently contaminated. He was greenish-looking and feeble for days before they ran any tests to determine why he wasn’t recovering well from the surgery (he was in great health at the time despite being in his early 80s- walked 3 miles daily). By the time they gave him intravenous antibiotics it was too late, he died about 24-36 hours later.
I’ve also heard horror stories about nurses’ treatment of female friends during childbirth.
Again those journalists with strange foreign names in the NYT.

I stopped reading sportswriters 10 years ago because too much of the time they just kiss up to jocks, coaches, and especially owners while adding no analysis. The TV sports channels are just “Watch 2 or 3 dudes with Q Score pedigree who are also good at arguing dominance games for the cameras” although now it is merging with the women’s klatsch shows.
I admit, I liked to watch “Capitol Gang” on CNN growing up but there was at least some understanding of who’s-up-who’s-down in external, non-TV politics you got from the interplay of the elderly male & female gladiators. Fractured his hip after he fell down slick porch stairs (it had been raining) visiting a friend. He was walking 3 miles before the accident, they opted for the hip replacement because they thought he was healthy enough that he could still benefit/recover from it. But apparently the surgical team couldn’t be bothered to sterilize the equipment, the new joint, or their hands properly (not sure which, but the point is he was infection-free before surgery).
Considering his father lived until his mid 90s, he probably had at least another decade in him.
Do you doubt it? Have you been in a hospital outside of a wealthy major metro and observed how unsanitary they are or how surly the staff can be?

Many, no doubt. A coworker of my husband died after botched intubation at a regional hospital. Don’t know that many details, but intubating her was probably unnecessary. I heard she was lucid/talking but uncomfortable when she was admitted. She’s actually the only person under 55 I know of that died of COVID or related complications.
This was also very early in the pandemic, around April. The period in which doctors were quick to intubate because they thought it prevented the “airborne infection” from spreading around the hospital.
How many people died due to unnecessary intubation? Someone should research it.

Leaving aside the differing tax treatment of dividends, the trick with dividends has always seemed to me to be to find companies who are likely to continue paying their dividends despite short-term difficulty that has driven down their share price. That maximizes dividend yield (of your investment; the “yield” on whatever the current market price is after you invest is irrelevant) while also allowing for capital appreciation gravy: win-win as they say.
Finding such situations is easier said than done though. The classic one I remember is the anti-tobacco hysteria in the 1990s when a bunch of states sued tobacco companies for something something billions dollar settlement something. Tobacco companies’ share prices all plummeted in the moral panic about it, but if you looked past the panic and fear it was obvious that 1) most smokers aren’t gonna quit, and 2) if the tobacco companies went out of business there would be no one for the states to collect from, so the states weren’t going to let that happen. Tobacco companies share prices were at historic lows, which also made their dividend yields absurdly high. It was a screaming “buy” signal, but almost everyone was too busy running the Tobacco Man Bad script to notice.
Any such situations today? Well, REITs typically have high dividends, and COVID + BLM has trounced urban real estate making any REIT specializing in that lower priced now. So if you think the current insanity is temporary, get an urban REIT and enjoy the rebound while you cash your dividend checks. (I don’t think the current insanity is temporary, so you’ll win or lose without me on this one. Also I’m broke, so you shouldn’t take my advice, lol!) I dunno. If you’re serious about this, just check a stock screener for the fifty or hundred highest yielding dividend stocks. Usually a lot of these will have had recent share price drops, which make their dividend yield reciprocally higher. Go down the list eliminating the ones who are in some kind of intractable trouble (often involves high debt) and looking for the ones whose problems are temporary but whose business is long term prospects are good. Then buy and hold.

If you want to make it a long term strategy, repeat the analysis every half year or year. You’ll probably see new candidates enter your target zone and others drop out.* Over time, it will also give some “feel” for that end of the market.

If you like the dividend, just keep holding. If you see better candidates, then you can sell and redeploy. If they drop out because they go belly up, obviously that’s not so good for them, but if you didn’t invest, that’s not your problem.

I’ve been noticing that in hospitals, the real power to make decisions is being pulled back, to positions higher in the organization chart.
One of the advantages of hiring someone who isn’t competent in their job is that they become dependent on people higher in the organization. They can’t decide what to do, and so they want someone to tell them what to do.
Real doctors trained fifty years ago don’t need anyone to tell them what to do. Every one of those guys were trained to be an independent decision-maker, and woe unto any administrator or higher-up who disputed their expertise.
Hiring an incompetent administrator or doctor for reasons of affirmative action has two advantages; first, you meet the current regime’s PC doctrine, and second, you have someone who just does what they’re told, because they’re not competent to do otherwise.
Have you been in a hospital outside of a wealthy major metro and observed how unsanitary they are or how surly the staff can be?
I’ve spent 85% of my life outside ‘wealthy major metros’ and I’ve been employed in hospitals. The surliness I’ve seen was in reaction to difficult patients, who are plentiful in emergency rooms. I can think of one exception, courtesy a neophyte lady doctor about 20 years ago when we’d called ahead. Now, I have seen bad behavior in ambulatory clinics – always by clerical staff. Even that’s not very frequent; I can think of about three instances in 40 years. I’ve never observed ‘unsanitary conditions’. The state medical center in Syracuse, ca. 2007 had some disconcerting features (e.g. security theatre and marks on the walls), but that’s not unsanitary. The small community hospital where I once lived (in a town with 4,000 people in it) had no noticeable problems with staff attitude or hygiene.
Someone has told you that medical negligence is the 3d leading cause of death in this country. I’d put that in the same file as I would one-in-five-college-women-are-sexually-assaulted. Assume someone’s scamming around with their taxonomy until they prove otherwise.
At my current place of work there is a black “doctor” who is the worst case of affirmative action I have ever seen. I mean, despite all my years of race realism, I never knew it could be this bad. Sure, I knew black medical school applicants were held to a lower standard, but all the black doctors I’ve met in the past seemed to at least meet the minimum level of competence. This is because in medicine, there’s a hard floor: the step exams. No matter how much a med school might want to let you in with lower grades or MCAT scores, you need to be able to pass those steps. And this person–I’m honestly at a loss as to how he managed to do so. He talks like what a mainstream conservative might call an “uneducated black person” (you and I know that really means unintelligent.) If you were standing next to him at the bus stop, you would think he was Quontavius from the ghetto who works at Walmart and has kids by 3 different mothers. The almost slurred-speech level of drawl, the difficulty finding words… his facial features even betray low intelligence. I’d peg his IQ at no more than 90. Overhearing him on the phone trying to talk to a patient’s family is like nails on a chalkboard. I can only imagine what patients think when they meet him. It’s actually quite exasperating and infuriating, especially since he gets away with shoddy work and doing whatever he wants and knows he’s unimpeachable because he’s black.
And it’s all the more so because ISTM that everyone else there has gaslit themselves into believing that he is intelligent and competent and this is normal. I’ve never had a behind-closed-doors talk, never even a knowing glance from a co-worker. As far as I can tell, I’m the only one who notices at all. It’s honestly enough reason by itself for me to want to leave the job, just to not have to see and deal with it.
Note, this is not just a cutesy point for ideological sparing.
The *only* way forward for conservatives and Republicans now, both ideologically and electorally is to take this issue head on and call bullshit on “structural racism”.
All manner of trying to square the circle or dodge it, will wake up no one, motivate no one and only lead to defeat. Exactly Trump’s failure.
Conservatives can toss in the obvious admission
— there once was “structural racism”–slavery and Jim Crow, and limited advancement opportunities in white organizations–but hasn’t been for 50 years
and supporting points
— various groups with various behavioral traits have varying success (ex. Jews, Hindus, Chinese, Mexicans, Cubans, various white ethnic groups, Yankee WASPs, Germans, Irish, Scots-Irish etc.)
But at root conservatives/Republicans must be willing to say:
— structural racism is bullshit
— white people have a right to live as white people–with their own norms, values, culture–in the land their ancestors built
— and blacks who feel oppressed by that … then basic logic dictates that they should have their own nation and live separately where whites don’t “oppress” them …
(that, of course, is not what that minoritarians want, what this is about)
From my experience, you really don’t want to end up in a hospital unless you have absolutely no alternative.
“Medicine” is a joke — full of a plague of Faucis bought off by Rockefeller “medicine”, Big Pharma and Gates — an independent review showed the CDC inflated CVirus deaths by 1600% by changing the rules just for CVirus they had in place for 17 years as to measuring deaths — and broke Federal Law in doing so, totally political “non-Science” activity — will anything happen? I was a research scientist/professor for over 40 years, published over 200 peer-reviewed papers in journals that include Nature and Science plus three books, supervised close to 20 graduate students and PDFs from western nations, China, Africa, India and Egypt, and lectured and gave short-courses all over the world. And I can say that there is both racism and sexism in Canada at the institutional level…but it is not against people of colour or women but against men of European backgrounds. In fact, there are even government funded programs that discriminate openly against white men.
This is medical nonentities (in the sense of personally having a direct hand in saving patients) like Fauci, minus the prime time slot, kvetching. People at the bottom of medical school classes need jobs, too. Those who can, practice. Those who can’t, join bureaucracies, where they snipe at those who can.
You know how Fauci acquiesced to Cuomo’s sequestering of pandemic patients in nursing homes? It’s proof positive that this conman is a better politician than he is a patient advocate. And in theory, Fauci is the best of the med school dregs. Pathetic does not begin to describe this overpaid incompetent.

Agree, and in the spirit of doing so, may I point out that
even slavery, by itself, is not “structural [or systemic] racism”, since there were free blacks in antebellum times and not all bondsmen were black.
Jim Crow too, is generally discussed as if it were worse than it actually was. The “Jim Crow” system was nothing but post-Civil War segregation and so applied to both blacks and whites equally: “separate but equal” as the Plessy v. Fergusson court decision upholding its Constitutionality concluded. What’s so horrible about blacks being with blacks while whites are with whites? It’s only horrible if you think there is something wrong with being with blacks, but this is what the anti-Jim Crow people are implicitly arguing, though they usually manage to dodge the implication of their own argument. Don’t let them.
All of this crying about structural racism, can be summed up in a few words…… “More black shucking and jiving!”
